How Harmony First can help

Patient advocacy & appointment preparation

Organize your concerns, prepare questions for your doctor, and feel more confident speaking up. We help you identify questions about your care team's instructions and next steps. Nonclinical appointment accompaniment may be available when requested and approved.

Caregiver & long-distance family support

Find education, resource connections, and practical guidance while caring for a loved one. We support family caregivers, including those coordinating care from a distance. Our Caregiver Support & Appreciation Program recognizes two selected caregivers each quarter.

Care coordination after a hospital or rehab stay

Make sense of follow-up instructions, organize next-step questions, and connect with appropriate community resources. Our Community Care Navigator Program provides nonclinical support through care transitions.

Healthcare & benefits paperwork

Get help understanding coverage information, explanations of benefits (EOBs), and medical bills. We provide general guidance and completeness review for Medicaid and SSI/SSDI application paperwork, plus questions to prepare for agency or insurance calls.

Community resource connections

Identify organizations that may help with food, housing, utilities, transportation, and other community needs. We help connect families with appropriate resources; assistance depends on each provider's availability and requirements.

Mental health navigation

Find referrals to licensed counselors and other appropriate support resources. Harmony First offers nonclinical navigation and listening support; we do not provide clinical mental health treatment.

Support for you and your family

We welcome seniors, family caregivers and Care Partners, veterans, adults with disabilities, people living with chronic illness or chronic pain, and families affected by Alzheimer's disease or dementia.

Harmony First operates virtually, with community support based on location and availability. Submit a request so we can discuss what can be provided remotely and whether in-person appointment support is available in your area.
